The new 2016-2020 Strategic Plan for Community Care City of Kawartha Lakes reflects input from a wide variety of individuals and organizations, and will guide the agency in meeting the important health and community support needs of the communities where we live and work. Quality Improvement Plan
The mission of Community Care is to, in partnership:
- Provide a comprehensive range of health and support services for people of all ages in the City of Kawartha Lakes;
- Engage the public in the pursuit of lifestyles that create a sense of community and improve quality of life.
The agency strives to create a safe, healthy environment where staff and volunteers work together to provide quality client care and services.
CCCKL defines "Quality" as the provision of care and services that are:
The organization's Quality Improvement Plan (QIP) demonstrates the agency's commitment to improve the quality of client care services that are delivered. The QIP allows for a systematic, co-ordinated and continuous approach to improving agency performance by focusing on the processes and mechanisms that address the above values. It also outlines the goals and strategies for ensuring client safety, delivering optimal care and achieving high client satisfaction. Finally, the QIP focuses agency improvement efforts on direct client care delivery processes and support processes that promote optimal client outcomes and effective business practices.
Each program area within the agency is responsible for implementing quality improvement (QI) activities. All QI initiatives are conducted as a part of the organization-wide Quality Management Committee activities.
Regular monitoring, quarterly reporting and annual evaluations of the QIP take place as part of the QI journey for CCCKL. Planning is done with a three-year horizon in mind, in alignment with the agency's Central East LHIN reporting requirements and the CCCKL Strategic Planning cycle. This may include the further development of existing goals, or the addition of new goals based on priorities and/or emerging issues that involve Community Care.
