The Community Care City of Kawartha Lakes Community Health Centre (CHC) is one of 108 CHCs in the province.

We are a member of the Association of Ontario Health Centres, the advocacy organization for non-profit, community-governed, multidisciplinary primary health care models in the province. 

The CHC provides comprehensive primary care services (family medicine), delivered by a team of professionals in order to promote healthy individuals and communities. Services we provide for our clients include: ongoing medical care, nutrition counseling and support, mental health counseling and therapeutic groups, diabetes education in collaboration with the Ross Memorial Hospital Health First Diabetes Education Team, and smoking cessation.

Application for CHC Services 

Local residents interested in applying for services from the Community Health Centre can access the CHC Application Form here. Please print the form and complete one form per family member. Return it to the Community Health Centre by mail, fax or in person. Our staff will contact you as soon as possible to arrange an appointment. 

 

 

Model of Health and Well Being

The CHC Model of Health and Well Being is the foundation of the care we provide. The model keeps people and community at the centre of our work and it recognizes that to influence health we must reach beyond our centre's walls to work on a number of different levels. See the Model below:
